If you have a printer, you can download your intake materials, print them, and complete them in advance, saving you time at your appointment. There are four required documents you must download:
Intake Packet Instructions – This two page document explains how to complete the intake packet and lists the tax documents and other information you need to bring to your appointment.
Can Tax-Aide Do My Taxes? – Print and complete this page first to determine if your tax situation has any factors that might prevent us from being able to prepare your tax return.
Intake Booklet– This 10-page form gathers important information that your Tax-Aide Counselor will use in preparing your tax return. You can complete it on your computer, then print it. Or you can print it and complete by hand.
Additional Intake Questions – This one page document gathers additional information for preparing your tax return. It also lets you know if your situation requires you to bring additional documentation that the preparer will need. Print this page and complete it by hand.
There are also two worksheets you can download if they apply to your situation:
Itemized Deductions Worksheet – Download this sheet if you think your itemized deductions (medical costs, property taxes, mortgage interest, charitable contributions) will be over $12,500 (filing single) or $25,000 (married filing joint)
Self-Employed Worksheet – Download this sheet if you are self-employed or do "gig work." It helps you gather the needed information about your income and expenses related to your self-employment.
